Grant Application

The Foundation will support programs and projects aimed at mitigating health issues prevalent in the hospitals' service areas. Such Projects and programs will be selected on the basis of their potential to make a demonstrable impact on the health status of the target community. Grants would be awarded on a competitive basis and must address a pressing healthcare need in the community.

To receive further consideration by the AHMC Health Foundation Board of Directors, please submit a funding proposal in accordance with the following requirements:

  • Grant Application and supporting documents listed below
  • All proposals should be typewritten and double-spaced
  • Incomplete proposals may be disqualified

The AHMC Health Foundation will evaluate proposals on a quarterly basis. Applicants will be notified of the board's decision within ninety (90) days of proposal submission.
